Being very familiar with those types of "simple sorting" formulae in non-medical settings, I've seen how corporations manipulate the semi-automated systems within which those formulae are deployed.
Constantly-moving statistical goalposts are the dystopian environment within which we all now live. You don't have to be a John Ioannidis or a Norman Fenton to grasp the basic principles underlying the mendacity of these systems.
My disabled Navy Vet buddy has been receiving pain (&stuff can't remember contents) injections between vertebrae on Non-VA medicare UH sopplemental plan. Last month, UH changed his long-standing pre-approved procedure and denied pre-shot anesthesia based on new insurance review as "unnecessary." He was on a gurney being rolled into the operating theater when the nurse told him.
16 spinal shots. i told my friend to get his doctor to vehemently object & recode -appeal -whatever will fix his plight before the next round.
The corruption runs so deep it is almost unfathomable. Remember these payers also basically forced the clot shots on people even though their statisticians were well aware of the side effects and lack of any benefit. Why? Because they are given a piece of the pie by the corrupt drug companies. Who are encouraged by the corrupt HHS. Who is further encouraged by the corrupt white house. There will be no consequences, they will find a corrupt judge and have this thrown out. When is enough, enough?
Garbage In / Garbage Out. Big surprise. All programs are filled with the bias of the folks that write them. That an insurance company would choose a program that would have a bias to deny claims should not be news to anyone. It is almost like UH had the intention to defraud their customers. They only bought the tools to accomplish the task and then put them into use. Why aren't we seeing the perp walks? This has RICO written all over it.

Anthem Blue Cross was sued years ago for denying claims (even if it was covered ) on the first request.
They realized that 40% of their clients would self pay!!!

The goal may be to dissolve insurance eventually. ( sooner than we expect)
